On July 12, 1973, a disastrous fire at National Personnel Records Center, located in St. Louis, MO; destroyed approximately 16-18 million Official Military Personnel Files.

If applying for benefits, you will be sent one (1) or two (2) forms to aid in determining eligibility.

NA Form 13075, Questionnaire About Military Service and/or
NA Form 13055. Request for Information Needed to Reconstruct Medical Data.

 The affected record collections are described below.

 RECORDS MISSING:

Army: Personnel discharged November 1, 1912, to January 1, 1960. Estimated Loss: 80%

Air Force: Personnel discharged September 25, 1947, to January 1, 1964 (with names alphabetically after Hubbard, James E) Estimated Loss: 75%
